Frequently Asked Questions

A DOT physical (also called a CDL physical or DOT medical exam) is a federally required health examination for commercial motor vehicle drivers. It’s mandated by the Federal Motor Carrier Safety Administration (FMCSA) to confirm you’re medically fit to operate a commercial vehicle safely. Passing the exam earns you a Medical Examiner’s Certificate (MEC) — the “medical card” you must carry to keep your CDL valid.

Our DOT physical is a flat $125, payable at your visit by cash or card. The fee is the same whether you’re certified for 24 months or a shorter period.

No. DOT physicals are an employment/certification requirement rather than medical treatment, so they are self-pay and not billed to health insurance — this is standard for DOT exams everywhere. Some employers reimburse drivers; keep your receipt if yours does.

To avoid delays or an incomplete exam, please bring:

  • A valid photo ID or driver’s license (and your current CDL or permit, if you have one)
  • Your current Medical Examiner’s Certificate (DOT medical card), if you have one
  • Glasses, contacts, or hearing aids, if you use them
  • A complete list of your medications (name, dose, and prescribing doctor)
  • If you have high blood pressure: recent readings or a clearance note
  • If you take blood thinners (e.g., warfarin/Coumadin): a recent INR result
  • If you have diabetes: a recent A1c and/or blood-sugar logs (insulin-treated drivers also need Form MCSA-5870 completed by your treating provider)
  • If you have sleep apnea: your CPAP compliance report (typically the last 90 days) and/or your sleep study
  • If you have a heart condition: a clearance letter from your cardiologist
  • If you’ve had a stroke, TIA, seizures, or fainting spells: a clearance letter from your neurologist
  • If you have significant vision problems or vision loss in one eye: FMCSA Form MCSA-5871, completed by your eye doctor
  • If you’ve had surgery in the past 12 months: a note from your surgeon clearing you to return to work
  • Any relevant specialist evaluations, waivers, or FMCSA exemptions

Bringing the right records is the single best way to avoid a “determination pending” result that would require a return visit.

The exam follows FMCSA standards and includes a review of your health history, vision and hearing testing, blood pressure and pulse, a urinalysis, and a physical examination.

Most exams take about 20–30 minutes if you bring the documentation listed above.

Up to 24 months. Drivers with certain conditions (such as high blood pressure, diabetes, or sleep apnea) may be certified for a shorter period so the condition can be monitored.

Not passing doesn’t necessarily mean permanent disqualification. Common issues — high blood pressure, uncontrolled diabetes, untreated sleep apnea, or vision problems — can often be addressed. In some cases we can issue certification once you provide additional documentation; in others you may need a follow-up visit or treatment before re-examination.

Yes — there is a link to schedule your appointment at the top of this page.

Your exam is performed by a Certified Medical Examiner listed on the National Registry of Certified Medical Examiners (NRCME), as required by FMCSA. An examiner who isn’t on the National Registry cannot legally issue your medical card.

No. The urinalysis in a DOT physical is not a drug screen. It checks for protein, blood, and sugar as part of the federal exam requirements. DOT drug testing is a separate process handled through your employer’s testing program.

No. Marijuana is prohibited for commercial drivers under federal law, regardless of Washington state law or a medical recommendation. This applies to all forms, including edibles and products containing THC.

Blood pressure is one of the most common reasons a driver receives a shorter certification period. Avoid caffeine, nicotine, and energy drinks right before your visit, and if you take blood pressure medication, take it as prescribed on the day of your exam.
